In the unfortunate event that you require your medication urgently please contact the dispensary or prescriptions department directly and speak to one of the dispensers who will advise you of when you can collect your medication. Alternatively when the surgery is closed please seek advice from the out of hours service or a local pharmacy, if it is possible to wait until the surgery re-opens please do so and contact us immediately, we will be happy to help.
By Telephone
Please telephone 01789 268905. Please leave your, Name, Date of Birth, Patient ID Number, Address and medications you require.
Personally
Please use a repeat prescription form at the desk or leave the tear-off counterfoil in the box provided at reception. If there is any query you may be asked to see the doctor. Prescriptions may be collected during surgery hours.
By Internet
Please use the form below to order your repeat prescriptions. You need only to quote your reference number (found at the top left of your previous prescription) and the details of the medication you require. Please make sure you select the appropriate surgery you would like to collect your prescription from.
Orders for Trinity Court will be prescrition format. Claverdon orderers will be dispensed for collection.
Prescriptions may be collected during surgery hours but not Saturday mornings.
Please allow 2 working days (48 hours) notice if possible.
